Pancreatic insufficiency [exocrine]: a dysfunctional pancreas WITHOUT the ability to make and RELEASE insulin via exocrine ducts into the small intestine or simply a pancreas that makes insulin but cannot release it due to inflammatory or structural issues blocking the flow of insulin to the gut.

Exocrine refers to release of a hormone or secretion directly into a lumen or cavity as opposed to release into blood which is referred to as, an endocrine secretion/release.
